This can be done on one list. Make sure if it is a business that the name of the business and owner is legible. If it is a resident make sure the name and address is legible. (Required of all applications) Copy of proof of insurance. (Minimum $1,000,000.00, required of all applications) Attachments If any items are being sold on City street or sidewalk then a Temporary Use Permit must be attached for each business participating. Permits can be obtained from General Services. State Food Service License if selling food. State Alcoholic Beverage License. (You can only serve on private property. No alcoholic beverages on City property, this includes streets and sidewalks) ~Clean-up is required within 24 hours. ~No alcoholic beverages can be consumed or taken out on City property, streets or sidewalks. ~No donations can be requested if any type of alcoholic beverages are served on private property/business unless you possess a State Alcoholic Beverage License. Please note there are inside consumption and outside consumption licenses. You must have the appropriate license. R'The City Public Works Department will deliver the appropriate barricades. Maureen Burroughs, PreSident of Okeechobee Main Street presented the First Ouarterly Report for 2007. What Have We Done? Sponsored second annual Green Market in the park beginning November 1,2006; Attended the Florida Main Street State Conference in Fort Pierce; Sponsored the Second Annual Christmas Window Contest - winner was Raulerson's Absolutely Art; Received a grant from Morgan Stanley in the amount of two thousand dollars ($2,000.00) to assist with the Juried Arts Fest and support of the various children programs sponsored by Main Street and the School Board; Presented Mural Guidelines to the County for adoption; Secured an agreement with Embarq to place a mural on the downtown Central Office Building; Conducted the Second Annual Halloween Festival in the park with assistance from various non-profit organizations and the City; Developed plans for First Annual Juried Arts Fest in the park for February 23-24, 2007; Received confirmation from the State on the Old Jail grant ranking us eighteenth on the list of over 140 grants for funding - money to be distributed in July 2007; Completed business survey for Local Program Area which will assist in determining needs of our local businesses; Completed the State Quarterly Report - to date OKMS has volunteered 14,206.5 hours from the beginning of the program, when multiplied by $6.15 hourly rate the value is $87,368.98 in-kind contributions to the Okeechobee Main Street program and to Okeehcobee; Sponsored Honey Baked Hams promotion (Thanksgiving and Christmas) as a community service; Provided an internet media advertising program (ShopMainStreet.org) for Main Street members (free of charge) to advertise their businesses; Conducted meeting with CSX representative to begin negotiations to secure ownership of old train station; Completed conceptual plan for Courthouse Park and presented to Board of County Commissioners; Established three new business members and two new individual memberships - this brings our total membership to 84 businesses and 38 individuals; Hired new Program Manager in December 2006.
