Mayor & Council, -the annual evaluation on the City Administrator is required to be completed
in July this year.

1. Your a blank, fillable evaluation form (names, review period and council meeting dates
have been filled in).
2. A copy of his current contract and job description.
A few reminders/information:
• After opening the PDF form you'll need to save it;
• To tally the overall score: add the individual question points, divide that number by 14
(total number of questions).
• You have the option of printing & sign this or email it back to me to print for you to sign
(print 1-sided only please).
• The original form is to be provided to me, before the start of the 7-19-2022
meeting to calculate the average score and give to the Mayor.
