2021-01-19 Ex 06EA I b,f//��//tp `,,,.Of• OKF1c�,y JM, IQ , 2b ZI CITY OF OKEECHOBEE Dis ibULd COVI D-19 LEAVE POLICY EFFECTIVE DATE: JANUARY 1, 2021 SUNSETS: SEPTEMBER 309 2021 All full-time and regularly scheduled part-time employees may be eligible per Fiscal Year for up to 10 calendar days of administrative leave with pay, due to: exposure of, quarantining from, experiencing symptoms of, or testing positive for COVID-19. A. Employee exposed on and/or off the job to an individual who has tested positive for COVID-19 (excludes employees who have had COVID-19 within the past 3 months); or the employee is experiencing COVID-19 symptoms, based on the most recent information provided by the Centers for Disease Control and Prevention (CDC). Employees who tested positive within the past three months and recovered do not have to quarantine or get tested again if they do not develop new symptoms. • If employee develops symptoms within three months of their first bout of COVID-19, they are to report this to their Supervisor/Department Head. The Department Head is to obtain the appropriate course of action (if any) from the Okeechobee County Health Department. B. Report exposure/symptoms to immediate Supervisor/Department Head and follow normal department protocols for being absent. , • Leave of Absence Forms are to be. completed and "furs charged to Administrative Leave, circle with pay and note to the side "COVID"; • The Department Head/Designee must ensure proper tracking of the number of days; • Department Headnotifies the City Clerk and City Administrator via email name of employee and date quarantine begins; • Employee is to begin their quarantine following the CDC guidelines, stay home, separate themselves from others, and monitor their health. C. If an employee elects to use COVID-19 Leave, they are required to submit for the Rapid in House COVID-19 Antigen Test. 1. Complete and`""n a ICOVID-19 Rapid Test Consent and Release Form, original provided to City Clerk's Offii;• 2. Department Head/Designee completes the 2Request to Urgent Care for Rapid COVID-19 Test Form, copy to City Clerk's Office, original to employee to provide to TCMA Urgent Care. The form provides instructions to TCMA and to invoice the City. 3. If the employee is not experiencing any symptoms, they are required to submit for testing no earlier than the fifth day and no later than the sixth day of quarantine. If experiencing symptoms, they are to monitor their health for 24 hours, then contact Urgent Care who will advise and schedule the appropriate date/time to conduct the test. 4. Test results are provided to the employee and to the City. Page 1 of 2 1,2 Forms attached, additional copies available from City Clerk's Office Negative Test Results: a. If the results are negative without symptoms, the employee is to return after day seven with strict mitigation measures (wear masks and social distance) through day 14. • Employee is to make immediate contact with their Supervisor/Department Head for instructions as to date/time to return to work. • It could be the same day as results are provided, or another shift depending on the needs of the department. b. If the results are negative and symptoms develop, the employee may request to be absent by using their accrued sick leave and re -test at their own expense at a facility of their choosing. • Should the second test result be positive the employee will need to provide the documentation to the Department Head, ; who will coordinate with the City Administrator to modify the Leave to Administrative Leave with Pay. • The employee then follows the procedures for positive results with symptoms. Positive Test Results. a. If the results are positive without symptoms, the employee is to isolate themselves and remain absent up to 10 calendar days on administrative leave with pay. They may return to work on the 1111 day with strict mitigation measures (wear masks and social distance) through day 14. �J • Coordinate with Supervisor/DepartmentXt`h�ad for instructions as to date/time to return to work. • It could be the same day as results are provided, or another shift depending on the needs of the department. b. If the results are positive with symptoms, the employee is to remain absent up to 10 calendar days on administrative leave with pay. They may return to work after at least 10 days since symptoms first appeared, and at least 24 hours with no fever without fever -reducing medication, and other symptoms are improving. They may return to work on the 11t" day with strict mitigation measures (wear masks and social distance) through day 14. • Coordinate with Supervisor/Department Head for instructions as to date/time to return to work. • It could be the same day as`r�suIts are provided, or another shift depending on the needs of the department. D. Any absence needed after 10 calendar days per Fiscal Year will be charged to the employee's accrued paid sick leave (or annual leave due to insufficient sick hours). E. Employees are not required to use the COVID-19 Leave Policy and may follow the standard protocol for use of sick leave. F. This policy may run concurrent with the Federal Family Medical Leave Act. G. This policy becomes effective January 1, 2021, and sunsets September 30, 2021, unless extended further by action of the City Council.
