HRSA Grant
The Hanley Foundation is one of two Florida agencies administering a HRSA Rural Communities Opioid
Response Planning grant this year. The grant is designed to assist local communities: Glades, Hendry,
and Okeechobee Counties, in identifying the gaps in opioid prevention, treatment, and recovery and
developing a strategic plan in conjunction with the state opioid consortium to address the identified
gaps including workforce and sustainability.
As an important stakeholder for your county, you and your agency will be vital in identifying and
addressing the opioid response needs for the community. As such, we would like to invite you to
participate in the local consortium to address the opioid situation in your county.
The first meeting will be held on August 15, 2019, from 12:30-2:30 PM at Hendry County Emergency
management — Emergency Operations Center, 4425 F/-80, Labelle, FL 33935. The meetings will rotate
monthly between Hendry/Glades County and Okeechobee County, so that we can best address the
needs of the individual communities while maintaining our consortium level activities.
The second meeting will be held on September 12th, 2019, from 12:00-2:00 pm at the Williamson
Center at IRSC 2229 NW 9th Avenue, Okeechobee, FL 34972. As part of the process, we will be asking
participating agencies to provide us with a Memorandum of Understanding outlining the expectations
and participation levels of each party. To assist us with the creation of the MOU format, please indicate
which of the following items below you feel that your agency would participate in (you can mark as
many as you like):
A) Assist with Gap Analysis by providing data and assisting with analysis
B) Assist with Strategic Plan

We are grateful to all of our community partners for their continued service to Florida's youth and look

Thank you in advance for your participation and support!
Beyond Boundaries in Rural Southern Florida
Memorandum of Understanding
Hendry Okeechobee Glades Opioid Consortium (HOGOC)
The purpose of the Health Resources and Services Administration (HRSA) Beyond Boundaries in
Rural Southern Florida (BBRSF) project is to support prevention of and treatment for substance use
disorders, including opioid use disorder (OUD). The overall goal of the program is to reduce the
morbidity and mortality associated with opioid overdoses in high-risk, rural communities by
strengthening the capacity of multi -sector consortia to address one or more of three focus areas at
community, county, state, and/or regional levels: prevention, treatment, and recovery.
This Memorandum of Understanding entered into between [Each Signatory Agency Identified Below]
and Hanley Foundation, outlines the details of the partnership between two entities through the one-
year HRSA Rural Communities Opioid Response -Planning (RCORP-Planning) grant period from June
1, 2019 through May 31, 2020. The BBRSF grant will benefit Hendry, Okeechobee, and Glades
counties therefore all members who participate in Hendry, Okeechobee, Glades Opioid Consortium
(HOGOC) must reside or provide services in at least one of the three counties to ensure that award
activities exclusively benefit populations in those counties.

the consortium, assist in meeting grant deliverables, assist in organizing HOGOC meetings
and teleconferences, chair and provide minutes for all HOGOC meetings, and be the local
key point of contact for the general public.

Project Director), currently serves as the Consortium Chair and facilitates the Consortium Meetings.
Consortium meetings will be held monthly rotating between locations in Hendry/Glades and
