SCOPE:
This Evaluation Report is issued under Rule 61G20-3 and the applicable rules and regulations governing t le use of
construction materials in the State of Florida. The documentation submitted has been reviewed by Robert P ieminen,
P.E. for use of the product under the Florida Building Code. The product described herein has been evaiiated for
compliance with the 6th Edition (2017) Florida Building Code, High Velocity Hurricane Zone (HVHZ) sectims noted

LABELING: Labeling shall be in accordance with the requirements of the Accredited Quality Assurance Agency noted
herein.
CONTINUED COMPUANCE: This Evaluation Report is valid until such time as the named product(s) changes, the referenced
Quality Assurance documentation changes, or provisions of the Code that relate to the product change. Acceptance of
this Evaluation Report by the named client constitutes agreement to notify Robert Nieminen, P.E. of any cI anges to
the product(s), the Quality Assurance or the production facility location(s). NEMO I etc. requires a complete eview of
this Evaluation Report relative to updated Code requirements with each Code Cycle.
ADVERTISEMENT: The Evaluation Report number preceded by the words "NEMO' etc. Evaluated" may be displayed in
advertising literature. If any portion of the Evaluation Report is displayed, then it shall be done in its entirety.

This is a building code evaluation. Neither NEMO ETC, LLC nor Robert Nieminen, P.E. are, in any way, t - Designer
of Record for any project on which this Evaluation Report, or previous versions thereof, is/was used forpermitting
or design guidance unless retained specifically for that purpose.
5.2 Fire classification is not part of this Evaluation Report; refer to a current Approved Roofing Materials D -ectory for
fire ratings of this product.
5.3 For steel deck installations, foam plastic insulation shall be separated from the building interior in accor-lance with
FBC 2603.4 unless the exceptions stated in FBC 2603.4.1 and 2603.6 apply.
5.4 The evaluation herein pertains to above -deck roof components; deck -attachment details pertain to 'as -tested'
conditions under Testing Application Standard TAS 114, Appendix J. Roof decks shall be in accordanm with FBC
(HVHZ) requirements to the satisfaction of the Authority Having Jurisdiction.
5.5 Fastener spacing for mechanical attachment of anchor/base sheet or membrane is based on a minimu n fastener
resistance value in conjunction with the maximum design pressure (MDP) listed for a specific system. should the
fastener resistance be less than that required, as determined by the Building Official, a revised fastene spacing —
prepared, signed and sealed by a qualified design professional — may be submitted. Said revised faster er spacing
shall utilize the withdrawal resistance value taken from Testing Application Standard TAS 105 and calwlations in
compliance with Roofing Application Standard RAS 117 or Roofing Application Standard RAS 137.
5.5.1 If mechanical attachment to the structural deck through lightweight insulating concrete is proposed, field
withdrawal resistance testing shall be performed to confirm equivalent or determine enhanced fastenir ; patterns
and density. All testing and fastening design shall be in compliance with Testing Application Standard TPS 105 and
Roofing Application Standard RAS 117 and/or Roofing Application Standard RAS 137. Calculatior3 shall be
prepared, signed and sealed by a qualified design professional.
5.6 For systems where specific lightweight insulating concrete is referenced, consult the current HVI-I_ product
approval documentation for specific deck construction and limitations. For systems where specific I ihtweight
insulating concrete is not referenced, the minimum design mix shall be for minimum 300 psi cellular mate ial.
5.7 For recover installations, the existing roof shall be examined in accordance with FBC 1521.
5.8 Perimeter and corner areas shall comply with the enhanced uplift pressure requirements of these areas. Fastener
densities shall be increased, as calculated in compliance with Roofing Application Standard RAS 1= and/or
Roofing Application Standard RAS 137 by a qualified design professional. *This extrapolation is not pe itted for
systems marked with an asterisk*.
5.9 All attachment and sizing of perimeter nailers, metal profile and/or flashing termination designs shall c nform to
Roofing Application Standard RAS 111 and applicable wind load requirements.

Re: Police Department roof replacement contract extension
Per the attached. memo, there has been a change in the specification for the Police Department roof
replacement. The chain:ge'in the specification ;is due to the contractor determining that the deck of the
PD roof is metal rather than plywood as -anticipated. The change in the specification is very minor with
slight changesin fastener size, felt, and granulate. There isno change. to thepricefor the project.
The contractor has requested a 21 -day extension as the new specification needs to be approved by the
manufacturer to assure that the 15 -year warranty is valid.
Given the possible potential for a rain delay, Public Works is requesting that the contract completion
date be extended by 60 days to account for any other possible dekays. The contractor will' only be
notified that the•requested 21 -day has been. granted.
The new contract end date will be October 26, 2018

Mr. Allen as to our conversation Tuesday morning about the changes of the re -roofing scope for ITB # PW 01-
00-04-1.8 The City of Okeechobee, Fl.
Description of changes to be made to improve the scope of roofing materials to meet manufactures(GAF)
specifications to qualify for the required 15 year warranty as stated in the bid document.
#1 No changes
#2 Changing of roof fasteners or deck screws from a #12 to #14 gauge
#3 Increasing the type# 4 felt to a type #6 fiberglass felt.
#4 Instead of using a modified bitumen white granulate, SCI will use modified bitumen white granulate (Fire
Insulated).
SCI is asking for a 3 week extension period for completion of the project. The original completion date was
August 27th 2018. This extension is for changing the permit process period and having the GAF
representative to look at and approve the changes in scope to give a 15 year manufacture warranty.
SCT is increasing the quality of the Police Department roofing surface at NO extra charge to the City of
Okeechobee.
I am attaching the original scope so you can compare the new changes to be made for
ITB # PW 01-00-04-18

Motion and second by Council Members Ritter and Clark to amend the Contract awarded to Painting and Pressure
Cleaning by Big Lake, LLC, for Bid No. PW 03-00-06-18 Pressure Washing and Painting City Hall, Fire Department,
and Records Retention Building as follows: waive the performance bond requirement; amend and clarify the
workers compensation requirement to read that Big Lake Painting and Pressure Cleaning, LLC may operate under
the Florida workers compensation exemption law, or outside workers must be hired through an agency with workers
compensation insurance.

Motion and second by Council Members Chandler and Clark to grant an extension of the Police Department Roof
Replacement Contract for Bid No. PW 01-00-04-18.
The Contract began June 28, 2018, with a completion date of August 27, 2018. However, after the forensic
investigation of the roof was concluded, it was discovered that the underlying roofing material is metal as opposed
to the assumed plywood. The specifications have to be modified; the price remains the same. The Contractor
requested extending the Contract completion by 21 -days in order to obtain approval by the manufacturer to assure
the 15 year warranty would be valid with the minor specification change. Additionally, there could be weather delays
and staff requested authorization to extend the project 60 -days if needed. Mayor Watford asked if there was any
additional discussion on the request for a 60 -day extension; there was none.

Council Member Chandler moved to award Bid No. PW 02 -00 -05 -18 -RB City Hall Door Replacement, in the amount
of $35,865.00 to Boromei Construction of Okeechobee; seconded by Council Member Clark.
The Re -Bid was let on June 29, 2018, with a mandatory Pre -Bid meeting held on July 10, 2018. The opening was
July 18, 2018. In addition to Boromei Construction, Meticulous Construction of West Palm Beach submitted a bid for
$79,000.00. Council Member Ritter expressed concern with the almost 50 percent price difference from the
opposing Bidder. Administrator MontesDeOca assured him research has been done on the discrepancies of prices.

Piece of concrete Public Works Director Allen displayed at the Council meeting regarding the portion of
