Motion carried unanimously. V. NEW BUSINESS —Secretary. A. Site Plan Review Application No. 17 -001 -TRC, Second Review, Phases One and Two, expansion to an existing nursing home facility located at 1646 Highway 441 North, Okeechobee, FL — Senior Planner. Application No. 17 -001 -TRC was first presented at the April 20, 2017, meeting. The action taken was to have the applicant resubmit revisions and additional documentation required. Planning Staff Report: The Okeechobee Health Care Facility is located on 10.95 vacant acres. To the North is Raulerson Hospital, to the East is Taylor Creek, to the South is Northeast 14th Street and to the West is Highway 441 North, (North Parrott Avenue), along with Sunshine State Surgical Center. The existing facilities encompass approximately 81,127 square feet to operate a 180 -bed TRC - June 15, 2017 - Page 1 of V. NEW BUSINESS CONTINUED. A. Site Plan Review Application No. 17 -001 -TRC, Second Review, continued. Planning Staff Report continued: nursing home with administrative and staff offices, kitchen, dining, social, physical therapy and laundry areas, required parking (145 spaces), loading zones, retention, and landscaping. The project is proposed to be completed in phases. Each phase will appropriately address the required landscaping, paving and parking regulations as provided on the drawings. Phase One consists of a 14,700± square foot building which will expand the facilities by 18 -beds known as the "West -Expansion," and create a trash compactor area removing the existing garbage dumpsters; a 7,200 square foot laundry facility and loading dock; and a 42,562 square foot dry retention area. Phase Two will now consist of expanding the kitchen by 2,600 square feet and adding two 5,000 square foot and one 7,200 square foot maintenance buildings. The proposed 16,000 square foot "East -Expansion" facility was removed, which reduced the lot coverage and the overall impervious surface coverage. Planning Staff's responses to the required findings are the property is designated Commercial on the Future Land Use Map and Heavy Commercial on the Zoning Map. Adequacy of public facilities appear to have been or will be met. Regarding potable water and sanitary sewer, the applicant had previously provided letters from the Okeechobee Utility Authority (OUA) documenting there is adequate capacity and facilities, The County has confirmed a considerable level of excess capacity available to serve the solid waste disposal needs. The Applicant supplied an engineer's report addressing drainage and other aspects of the proposed project, no sidewalks are required. Planning Staff noted during the first TRC review, one of the stipulations was to submit a traffic impact analysis for the entire project including documentation on the current traffic levels being experienced by the existing nursing home facility as well as the projected traffic for all development envisioned for the project at buildout. The Applicant's representative has submitted only a cursory analysis using the Institute of Transportation Engineers (ITE) Traffic Generation Manual estimates for the "West -Expansion". In addition, he has stated since the "East -Expansion" was removed and the residual peak traffic impact for the nursing home at buildout is 44 trips, this is less than required for a full traffic study. Mr. Brisson stated ITE generation rates are only estimates based on some studies of other nursing homes. When there is an existing facility that is being expanded, traffic projections are best based upon the actual operation of the existing facility. The Applicant was to provide counts of vehicular traffic entering and exiting the site during the peak hours of operation of the facility as it exists now, thereby providing a basis for estimating the likely traffic generation for future expansions. The removal of the "East -Expansion" does not justify varying from the TRC's stipulation. It is clear that sometime in the future Parcel two, (refer to April 20, 2017 Minutes), will be put to more productive use and information should be provided that can be used in assessing potential traffic levels. The comparison of proposed and required dimensional standards found that all areas are adequate with the exception of one that was highlighted and listed as follows: 216 total parking spaces are planned for both phases of which nine are handicap spaces. However, at least one handicap space is to be provided for the maintenance building facilities. The following comments received by Secretary Burnette from Administrator Montes De Oca were read into the record by Building Official Newell. "Potential existing traffic and patterns shall be identified. Although the building was removed from the plans, the existing and potential traffic numbers should be noted and presented as per recommendation by Planner. I do not have additional comments and would be in support of the approval pending the above". TRC - June 15, 2017 - Page 2 of 3 V. NEW BUSINESS CONTINUED. A. Site Plan Review Application No. 17 -001 -TRC, Second Review, continued. County Environmental Health Department: No issues were received. OUA: Director Hayford commented all his requests have been satisfied and all easements issues have been handled. Public Works: Public Works Director Allen commented he still has concerns with the traffic study not being provided as we do not have a clear understanding of the actual traffic levels for the current and future uses. Police Department: Police Chief Peterson commented his Department researched and found no traffic accidents and no current issues at the proposed location. Building Official: No additional comments. Fire Department: Fire Chief Smith commented all his requests have been satisfied. Building Official Newell asked whether there were any comments or questions from those in attendance. Mr. Wendell Hall, P.A. with Bhide & Hall Architects, was in attendance. Mr. Newell commented the City has not received any building permits for the foundation as of yet. Mr. Hall responded he was working on submitting for that soon. Building Official Newell then asked TRC Members to disclose for the record whether they had spoken to anyone regarding the application or visited the site. There were no disclosures. The Building Official requested the action to be made in two separate motions. One regarding the approval or denial of the application without a traffic impact analysis and the other for the actual expansion. A motion and a second was offered by Police Chief Peterson and Fire Chief Smith to approve Site Plan Review Application No. 17 -001 -TRC, Second Review, Phase One only without a formal traffic impact analysis and to use the data presented until Phase Two of the project at which time a formal analysis would be required. Motion carried unanimously. Site Plan Review Application No. 17 -001 -TRC, in regards to the construction of a 15,000 square foot, 18 bed West expansion, a 7,200 square foot laundry building and a 2,600 square foot building for additional kitchen facilities for an existing nursing home facility located at 1646 U.S. Highway 441 North, Okeechobee, Florida - Senior Planner. TRC - April 20, 2017 - Page 1 of 4 lk VI. NEW BUSINESS CONTINUED. A. Site Plan Review Application No. 17 -001 -TRC continued. The property owner obtained site plan approval for Application No. 15 -004 -TRC on July 16, 2015. The West parking area, and highway entrance expansion was completed. However, the remainder of the project has been modified; this application will supersede the previous one approved. Planninq Staff Report: Planner Brisson began by commenting that the site plan submitted was confusing and required several conversations with the Project Engineer for clarification. The Okeechobee Health Care Facility is located on 10.95 acres, comprised of four unplatted parcels; including two annexed in April 2015. These four parcels are located as Parcel one 2.965± acres, is vacant and the northernmost parcel, lying East of Raulerson Hospital and West of Taylor Creek. Parcel two is 2.035± acres, is vacant and lies directly South of Parcel one, bounded by Parcel three to the West, Taylor Creek to the East, and Northeast 14th Street to the South. Parcel three is 4.319± acres, containing the majority of the facility, it is bounded by Parcel four to the West, Parcel two to the East, Raulerson Hospital to the North, and Northeast 14th Street to the South. Parcel four is 1.628± acres and houses the recently construed West parking area, it is bounded by U.S. Highway 441 North to the West, Parcel three to the East, Raulerson Hospital to the North and Northeast 14th Street to the South. Construct a 16,000 square foot facility on Parcel two that will expand the facility by another 18 -beds, and offices for staff, to be known as the "East -Expansion." Then on Parcel one, construct two 5,000 square feet and one 7,200 square foot maintenance buildings. Planning Staff's responses to the required findings are the property is designated Commercial on the Future Land Use Map and Heavy Commercial on the Zoning Map. Adequacy of public facilities appear to have been or will be met. Planning Staff noted a few items related to traffic generation and parking considerations in that the project will not have an effect on the level of service on U.S. Highway 441. However, should any parking space(s) or areas at the existing main facility that were devoted for laundry and/or maintenance activities change for other purposes such as additional beds, medical offices or out- patient services, such changes will need approval by the TRC or addressed through a Special Exception. TRC - April 20, 2017 - Page 2 of 4 VI. NEW BUSINESS CONTINUED. A. Site Plan Review Application No. 17 -001 -TRC continued. Additionally should the East -Expansion be used for any purposes by medical staff other than those directly associated with the nursing home, the site plan may have to be revised to address traffic generation and parking needs. The comparison of proposed and required dimensional standards found that most areas are adequate. Those that were not met were highlighted and listed as follows: 16 feet separation required between the two 5,000 square foot maintenance buildings, and between the new laundry facility and the East -Expansion buildings. Off-street parking, 241 total are planned, with eight being labeled handicapped. At least one handicap space is to be provided to serve the facilities proposed for Parcels one and two. A fourth loading zone is required and must be shown on the site plan. The driveway width for the parking aisle to the four parking spaces shown for Phase One on Parcel three has to be increased from 20 to 24 feet wide. The applicant needs to provide a specific landscaping plan suitable for scaling, so that staff can confirm the square footage of landscaped area. A photometric lighting plan conforming to City Code Section 78.71(A)(5) is to be submitted and reviewed. Mr. Steven Dobbs of SLD Engineering, LLC, was present and responded to various questions, noting the impervious information was included on the plans for Phases One and Two so that it would not have to be re -addressed. The drainage required for both Phases will be constructed in Phase One. The Architect is making final adjustments to the buildings, and once completed, he will be able to provide a site plan showing the crosswalks and striping. He confirmed a South Florida Water Management District permit was obtained. Administrator MontesDeOca noted as well that the proposed site plan was very confusing and asked several questions to clarify what would be built with which Phase and its location. He requested Mr. Dobbs provide water line information and potential looping for pressure, the location of fire hydrants and backflow prevention devices for future projects. Finally, that a traffic impact analysis is now needed based on the current and proposed uses. County Environmental Health Department: No issues were received. OUA: Director Hayford requested plans with the existing water and wastewater mains, a breakdown of employees listing existing, Phase One, Phase Two, and at peak demand. Provide water fixture counts on Phase one buildings in order to provide separate meters for each building, and to determine whether an easement is required. Noting the OUA and the Health Care Facility's Attorneys are researching to determine who owns the existing water main and whether an easement is required to maintain it since the entrance to the facility is a private drive. Public Works: No issues were received. Police Department: No issues were received. Building Official: Mr. Newell also asked questions to clarify his understanding of the specific proposed construction for each Phase and on which parcel. Fire Department: The age and condition of the fire service water main came into question based on the future projects that will require additional fire sprinkler protection and the ability of the system to provide a constant water source for fighting fires. Site Plan Review Application No. 17 -001 -TRC continued. not looped, which means that a damaged line could shut down operations while a looped system would at least have the possibility of water being supplied from a different direction. A fire engineer submitted with the previous site plan review, a letter confirming a looped system is not needed. Chief Smith stated an 8 -inch main is needed through the entire property. He also addressed that landscaping be kept trimmed in a manner to prevent low hanging branches across the driveway to the facility. In closing, Chief Smith added a safety concern related to the location of the Raulerson Hospital helicopter pad to the nursing home's driveway/access. His recommendation was some sort of system or device to prohibit vehicles from accessing the driveway from the facility's parking areas or from North Parrott Avenue, during the periods of time when emergency helicopters are landing or taking off. Chairperson Montes De Oca asked whether there were any comments or questions from those in attendance. There were none. He then asked TRC Members to disclose for the record whether they had spoken to anyone regarding the application or visited the site. There were no disclosures. A motion and a second was offered by Building Official Newell and Public Works Director Allen to have the applicant resubmit for the June 15, 2017 TRC Meeting for Site Plan Review Application No. 17 -001 -TRC. Committee Members are to compile their individual stipulations, email them to Board Secretary Burnette to forward to the Applicant for revisions and/or additional documentation to be submitted to General Services by May 18, 2017. The Applicant previously received approval in 2015 of a site plan for the "East Expansion" of the nursing home facility, including the two easternmost parcels which were annexed into the City to facilitate the expansion. That approval also included the two 5,000 -square foot maintenance buildings to be located on Parcel 1 in Phases 2 and 3. The overall site plan now includes a 7,200 -square foot maintenance building on Parcel 1 _ ? I Per the May 18th transmittal letter, the new site plan is proposed to include a 15,000 -square foot, 18 -bed "West Expansion" increasing capacity to 198 beds. The site plan also includes a 7,200 -square foot laundry building and a 2,600 -square foot building devoted to expanded kitchen facilities. The existing garbage dumpsters will be removed. The City of Qkeechobee requires one parking space for every four beds and one space for every employee at peak shift. The proposed expansion will increase the number of beds to 198 in the nursing home. There will be 123 employees at peak shift, requiring 173 parking spaces onsite. There will be 9 handicapped parking spaces around the facility, exceeding the ADA requirement of seven spaces for a 173 -space parking facility. The total area of the four parcels comprising the sitithe s 10.95 acres. With the proposed deletion of the 16,000 square -foot building on Parcel total building area of structures associated with the proposed new development is 2.21 acres, resulting in a lot coverage of 20.2%. Of the total acreage, this submittal is for a drainage area of 8.81 acres of which 5.07 acres is impervious or 57.5% appreciably less than the maximum 85% allowable. The applicant is planning to use the existing access to the site from the west which was previously expanded through FDOT to a 16 -foot wide entrance lane and a 12 -foot wide exit lane. SOLID WASTE DISPOSAL: On several occasions the County has confirmed a considerable level of excess capacity available to serve the solid waste disposal needs of other developments in the City. It appears reasonable that the volume of solid waste associated with a development of this size can also be accommodated within the capacity of the County's Solid Waste Facility. DRAINAGE: The Applicant has submitted, as part of the Application submittal, an Engi- neer's Report for the West Wing Expansion prepared by Steven L. Dobbs Engineering, LLC which addresses drainage and other engineering aspects of the proposed project. TRAFFIC GENERATION AND ACCESS CONSIDERATIONS: TRAFFIC GENERATION: The traffic generation factor for a nursing home cited in the ITE 8th Edition of Traffic Generation is 0.22 trips per bed in the PM "peak hour". This translates to an additional four trips during the PM Peak Hour for the 18 new beds being added in the planned expansion The current proposal also includes a 2,600 -square foot expansion of the kitchen facilities on Parcel 3, all to be constructed in Phase 2. However, during the initial TRC review in April one of the stipulations included in the TRC decision to require resubmittal of the site plan was: "A traffic impact analysis for the entire prosect to include documentation on the current traffic levels being experienced by the existing nursing home facility as well as the proiected traffic for all development envisioned for the project at buildout." In an e-mail to me on May 181' the Applicant's representative stated "...we have removed the 16 000-sf building to the east and the residual peak traffic impact for the nursing home at buildout is 44 trips. Less than that would be required for a full-blown traffic study." The subsequent Okeechobee Nursing Home Traffic Statement submitted to the City provides only a cursory analysis using the ITE Traffic Generation Manual estimates for the addition of an 18 -bed nursing home. The TRC required documentation on the actual traffic levels being experienced by the existing nursing home facility so that the City would have a clear understanding of how the existing facility is operating and the actual traffic generation characteristics of the nursing home and associated facilities. ITE generation rates are only estimates based on some studies of other nursing homes. When there is an existing facility that is being expanded, traffic projections are best based upon the actual operation of the existing facility. The idea behind the stipulation was that the Applicant would provide counts of vehicular traffic entering and exiting the site during the peak hours of operation of the facility as it now exists, thereby providing a basis for estimating the likely traffic generation of future expansions or other development of the site. The removal of the 16,000 -square foot building from the current proposal does not justify varying from the TRC's stipulation. ITE estimates do not provide "documentation on the current traffic levels being experienced by the existing nursing home". Furthermore, it is clear that Parcel 2 will, at some time in the future, be put to more productive use and information should be provided that can be used in assessing potential traffic levels in the future. Traffic associated with the expansion of the kitchen facilities is included in the traffic generation factor for a nursing home. Kitchen employees are included in the Applicant's projected employment numbers and parking needs. ACCESS AND EGRESS: The applicant will continue to use the existing 20 -foot wide private driveway to access the site from the west. This access way has been improved with a safety upgrade approved by FDOT expanding the facility to accommodate a 16 -foot wide and a 12 -foot wide exit lane. Review of the property with current buildings on site and anticipated additions and concluded that the existing water supply and pressure of the fire line to be adequate for both current and future fire protection needs. The available water supply and pressure is more than adequate without looping the system for fire protection for existing and future anticipated building additions. This project requires a minimun flow rate of 400 GPM at 20 PSIG at the sprinkler heads. The latest census attached shows 107 people onsite, with the additional 16 nurses and CNA's for the new west D wing, the peak would be 123 with a breakdown of approximately 16 nurses and CNA's for each wing with the remainder being maintenance, administration, food service, and housekeeping. The remaining 58 made up of administration, food service, and housekeeping are housed in between A (most easterly building) and B (next building to the west) wings, but are in all areas of the nursing home during the day. What is the use to be for the 16,000 sq ft building that was to be used for offices? The 16,000-sf building has been removed from the site plan for consideration with this submittal. Herb Smith, Fire Chief: Note: The age and condition of the fire service water main came into question based on the future projects that will require additional fire sprinkler protection and the ability of the system to provide a constant water source for fighting fires. The entire complex has only one fire main and it is NOT looped (which means that a damaged line could shut down firefighting operations while a looped system would at least have the possibility of water being supplied from a different direction). This subject has come up in the past site plan reviews in which a fire engineer submitted a letter confirming that a looped system is not needed. Requested the fire line to be evaluated of its condition in regard to the proposed plans. According to PIPA (Plastics Industry Pipe Association) article attached states a pvc pipe should be expected to last 100 years or more, however, even at the stated 50 -year life of a pvc pipe this pipe would be expected to last until 2034 with the installation in 1984. There is no reason to expect the existing pipe with no noted line breaks will not function to at least the 50 -year life expectancy. 23. Ensure fire main remains a minimum of 8 inches when extending on property. Please see attached the information provided to the owner when the line was installed that states the watermain is 8" onsite. 24. Be aware of future landscaping in regard to branches, etc. extending into roadways (accessibility of the emergency vehicles). Noted. 25. Due to the traffic congestion, currently on site and the potential of additional traffic, a request is being made from our department in reference to protecting the area near the helicopter pad. Presently, the pad used by the helicopter for the hospital is quite close to the egress road on the OHCF property. Although drivers attempt to proceed with caution during lift offs and landings, there are still many drivers who continue to drive or stop near the pad during this time which complicates matters and safety of all involved. The fire department would request some sort of system or devices used during the lift offs and landings to help keep the area safe and alleviate stressful landings and take offs for the pilot. There was a meeting held onsite on May 11, 2017 with the hospital and the nursing home and hospital have agreed to place a plastic chain across the drive. This chain will extend across the entire drive and will be placed and removed by hospital employees assigned to assist with the helicopter operation. Life Expectancy for Plastics Pipes Based on the use of 50 year stress regression data, it has been incorrectly assumed that plastics pipe systems have a life expectancy of 50 years. In reality, such systems can reasonably be expected to last 100 years or more. PE and PVC pipes and fittings were introduced into Australia during the 1950's, mainly for water supply and irrigation, but also for fuel gas and industrial applications. The first Australian Standard for PE pressure pipes was ASK119-1962 and the first for PVC pressure pipes was ASK138-1963. The creep rupture characteristics of these materials necessitated a new method for selection of design stress, compared with other materials in use at the time - AC, Cl, GWI, etc. The method adopted was that already in use in Europe - using the creep rupture (stress regression) curve, select a time and establish the associated burst stress. Apply a design factor to the burst stress to give the design stress. The time chosen Was 50 years, already adopted in Europe, and is still in use today in AS/NZS, ISO, and CEN Standards. The use of this particular time interval has led to the misunderstanding that it represents the pipe life. Similarly, the use of 50 year modulus values for use in ring deflection calculations for non -pressure pipes has also led to misunderstanding regarding life. The following extract from WSA01-2004, Polyethylene Pipeline Code, explains why prediction of system life should not be based on the arbitrarily chosen time value. PRESSURE Selection of allowable stress is based on long term pressure testing in the laboratory and regression analysis applied to the data obtained. The 50 year point is arbitrarily chosen for this basis, as for all thermoplastics pipes. A factor is applied to the 50 year point in order to provide the design stress. It shall not be taken that either: (a) the pipes weaken with time; or (b) the predicted life is 50 years. System life is dependent on many factors. If the design stress were used in relation to the regression curve, predicted pipe life would be indefinite, not 50 years. As with other materials, the life is dependent on manufacture, transport, handling, installation, operation, protection from third party damage and other external factors. Provided that PE pipeline system components are appraised and supplied to nominated industry standards under third -party product certification systems, and provided pipelines are designed and constructed correctly, then the likelihood of failure is minimised. For correctly manufactured and installed systems, the actual life cannot be predicted, but can logically be expected to be well in excess of 100 years before major rehabilitation is required. If a system life is to be assigned beyond 100 years, it has to be based on the likelihood of failure arising from the above factors, not the pipe regression curve. Pipe strength has been shown not to decrease with time -in fact, it increases slightly. "Instantaneous " burst pressure after a period in service will be at least equal to that of new pipe. Polyolefins Technical Information NON -PRESSURE TN013 The life of non -pressure PE pipelines will be dependent on performance under four main conditions: (a) Soil mechanics and pipe mechanics stability. (b) Pipe material strength (c) Chemical and biological stability. (d) Functional stability. The life of thermoplastic non -pressure pipeline systems has been extensively studied and reported. For example, the report titled Plastics Pipes -How Long Can They Last, by Prof Lars -Eric Janson of VBB Sweco Consulting Group reaffirmed a 1987 report concluding that the answer to the above question was "at least 100 years". The latest report, produced in 1996, states that "...it has been clearly found that nothing has emerged, which contradicts the statement made in 1987." It also states that the report refers mainly to buried gravity sewer pipes, but the conclusions can in most cases be applied for pressure applications. The aim of the work was to verify the claim of "at least 100 years". The summary states that "...one can thus conclude that everything is pointing to at least 100 years practical service life for today's buried sewer pipes made of high quality virgin PVC -Ll and PE resins, on condition that the pipes are used in accordance with the prevalent national standard installation instructions." Contractor is responsible for checking actual site conditions before starting construction. 2. Erosion and sedimentation Control techniques shall be incorporated during construction as follows: (1) silt screens shall be maintained at the project perimeter. (2) No off-site discharges shall occur during construction. In the event discharge is required, hay bales and/or turbidity curtains shall be incorporated at the discharge point as necessary to control turbidity. EROSION AND SEDIMENTATION CONTROL NOTES Construction activities can result in the generation of significant amounts of pollutants which may reach surface or ground wafers. One of the primary pollutants of surface waters is sediment due to erosion. Excessive quantities of sediment which reach water bodies of floodplains have been shown to adversely affect their physical, biological and chemical properties. Transported sediment can obstruct stream channels, reduce hydraulic Capacity of water bodies of floodplains, reduce the design capacity of Culverts and other works, and eliminate ethic invertebrates and fish spawning substrates by siltation. Excessive suspended sediments reduce light penetration and therefore, reduce primary productivity. MINIMUM STANDARDS: 1. Sediment basin and traps, perimeter dikes, sediment barriers and other measures intended to trap sediment shall be constructed as a first step in any land -distributing activity and shall be made functional before unslope land disturbance takes place. 2. All sediment control measures are to be adjusted to meet field Conditions at the time of construction and be constructed prior to any grading or disturbance of existing surface material on balance of site. Perimeter sediment barriers shall be Constructed to prevent sediment or trash from flowing or floating on to adjacent properties. 3. Permanent or temporary soil stabilization shall be applied to denuded areas within seven days after final grade is reached on any portion of the site. Temporary soil stabilization shall be applied within seven days to denuded areas that may not be at final grade but will remain undisturbed for longer than 30 days. Permanent stabilization shag be applied to areas that are to be left undisturbed for more than one year. 4. During construction of the project, soil stockpiles shall be stabilized or protected with sediment trapping measures. The applicant is responsible for the temporary protection and permanent stabilization of all soil stockpiles on site as well as soil intentionally transported from the project site. 5. A permanent vegetative Cover shall be established on denuded areas not otherwise permanently stabilized. Permanent vegetation shall not be considered established unfit a ground cover is achieved that, in the opinion of the Reviewer, is uniform, mature enough to survive and will inhibit erosion. 6. Surface runoff from disturbed areas that is comprised of flow from drainage areas greater than or equal to three acres shall be controlled by a sediment basin. The sediment basin shall be designed and Constructed to accommodate the anticipated sediment loading from the land -disturbing activity. The outfall device or system design shall take into account the total drainage area flowing through the disturbed area to be served by the basin. 8. After any significant rainfall, sediment control structures will be inspected for integrity. Any damaged devices shall be Corrected immediately. 9. Concentrated mnoff shall not flow down cut or fill slopes unless Contained within an adequate temporary or permanent channel, flume or slope drain structure. 10. Before temporary or newly constructed stormwater Conveyance channels are made operational, adequate outlet protection and any required temporary or permanent channel lining shall be installed in both the Conveyance channel and receiving channel. 13. When work in a live watercourse is performed, precautions shall be taken to minimize encroachment, Control sediment transport and stabilize the work area to the greatest extent Possible during construction. Nonerodible material shall be used for the Construction of causeways and Cofferdams. Earthen fill may be used for these structures if armored by nonerodible cover materials. 14. When a live watercourse must be Crossed by Construction vehicles, a temporary stream crossing Constructed of nonerodible material shall be provided. 15. The bed and banks of a watercourse shall be stabilized immediately after work in the watercourse is completed. 16. Periodic inspection and maintenance of all sediment Control structures must be provided to ensure intended purpose is accomplished. The Developer, owner and/or Contractor shall be Continually responsible for all sediment leaving the property. Sediment Control measures shag be in working Condition at the end of each working day. 17. Underground utility lines shall be installed in accordance with the following standards in addition to other applicable criteria: A. No more than 500 linear feet of trench maybe opened atone time. B. Excavated material shall be placed on the uphill side of trenches. C. Effluent from dewatering operations shall be fhered or passed through an approved sediment trapping device, or both, and discharged in a manner that does not adversely affect flowing streams or off-site property. D. Restabilization shall be accomplished in accordance with these regulations. 18. Where construction vehicle access routes intersect paved public roads, provisions shall be made to minimae the transport of sediment by tracking onto the paved surface, where sediment is transported onto a public mad surface with Curbs and gutters, the road shall be cleaned thoroughly at the end of each day. Sediment shall be removed from the roads by shoveling or sweeping and transported to a sediment control disposal area. Street washing shall be allowed only after sediment is removed in this manner. This provision shall apply to individual subdivision lots as well as to larger land -distributing activities. 19. All temporary erosion and sediment Control measures shall be removed within 30 days after final site stabilization or after the temporary measures are no longer needed, in the opinion of the Reviewer. Disturbed soil areas resulting from the disposition of temporary measures shall be permanently stabilized to prevent further erosion and sedimentation. 20. Properties and waterways downstream from construction site shall be protected from sediment disposition and erosion. 21. Phased projects should be cleared in Conjunction with construction of each phase. 22. Erosion Control design and Construction shall follow the requirements in Index Nos. 101 102 and 103 of FDOT Roadway and Traffic Design Standards. 23. The Reviewer may approve modifications or alter plans to these erosion Control criteria due to site specific Conditions. Clearing and Grubbing: Clearing and grubbing shall be performed within the limits of the project work in accordance with Section 110, Florida Department of Transportation (FDOT) Specifications. This item shall include, but is not limited to, the complete removal and legal disposal of all trees, r Undesirable material to a depth of 18 inches brush, stumps, roots, grass, weeds, rubbish and oche d p Wow natural ground or proposed finished grade, whichever is lower. The areas to be cleared generally consist of the entire site with the exception of areas specifically noted on the landscape plans as preserve areas or as areas to remain un -cleared. Care shall be taken to insure that no preserve areas or wetland areas are impacted by the clearing operation. Prior to initiating the tearing operation, all adjacent wetland and preserve areas shall be marked and Flagged in accordance with the City of Okeechobee and South Florida Water Management District (SFWMD) requirements All such areas immediately adjacent to the clearing operation shall also be protected by the installation of temporary silt barriers in accordance with the requirements of The City of Okeechobee and the SFWlv1D. Further erosion control shall be accomplished by seeding and mulching all disturbed areas as soon as they are at final grade, per the specifications for seeding and mulching found elsewhere on this sheet. All material shall be removed from the site and shall be legally disposed of in accordance with all local, state and federal requirements. 1. Earthwork and Grading: All earthwork and grading shall be performed as required to achieve the final grades, typical sections and elevations shown on the plans. In all other respects, materials and construction methods for earthwork, embankment, excavation and grading shall conform to the requirements of FDOT Specifications, Section 120. Any plastic or otherwise undesirable material within 36 inches of finished road grade shall be removed and replaced with suitable material. The contractor shall also refer to the Soils Report, if available. The specifications and recommendations included in that report shall be considered as a pan of these plans and specifications. Should there be any conflict behseen that document and any requirements of these drawings or specifications, the most restrictive requirement shall govem. 3. Paving Improvements: All areas proposed for paving shall be Constructed in accordance with the design grades and typical sections shown on the drawings, and in conformance to the requirements of the City of Okeechobee and Florida Department of Transportation. A. Asphalt: Prime Coat and tack coat for base course and between lifts of asphalt shall conform to the requirements of Sections 300-1 through 300-7 of the FDOT Specifications, Prime Coat shall be applied at a rate of 0.25 gallons per square yard and tack coat at a rate of 0.10 gallons per square yard unless otherwise approved by the Engineer. Asphalt surface course thickness and material shall be as shown on the typical sections and shall in all ways Conform to the requirements of FDOT. B. Base: Limerock base material shall be compacted to 98% of maximum density per AASHTO T-180. All limerock shall meet the minimum requirements of FDOT Section 911. As an alternate, cemented coquina conforming to FDOT Section 915 may be substituted mid shall be subject to the compaction specifications detailed above and included in the Soils Engineer's report. C. Sub -grade: Sub -grade shall be compacted to 98% of maximum density per AASHTO T-180, and stabilized to a minimum FBV of 50psi. Sub -grade shall be thoroughly rolled with a pneumatic tired roller prior to scheduling any sub -grade inspection D. Valley Gutterl F-Curb/D-Curb/Flush Curb: Shall be constructed per the typical section by extruding machine or forms as shown on the plans. Minimum concrete compressive strength shall be 3.000psi after 28 days. Sub -grade shall be moistened at the time Concrete is placed to insure a uniformiy damp surface. Ready -mix concrete shall have a slump of between 2 and 4 inches. No water shall be added to increase workability. Test cylinders shall be made for the strength testing of each batch of concrete for at least 7 and 28 dry testing. E. Sod: A minimum of a two -foot wide strip of sod or as otherwise shown on the plans, shall be placed along the back of curb of all constructed pavement to aid in prevention of erosion and soil stability. Sod shall be placed in conformance to FDOT Section 570, 575 and 981, Generally. the sodding requirements shall be as specified on the landscape plans, prepared by Others. F. Seed, Fertilize and Mulch: All disturbed areas shall be stabilized with seed fertilizer and mulch upon completion and acceptance by Engineer of final grading. Seed, fertilizer and mulch shall be in conformance to FDOT Sections 570, 575 and 981. The Contractor is responsible for establishing a stand of grass sufficient to prevent erosion prior to removal of the temporary silt fences. This applies only to those areas not covered by the sodding specified in the landscape plans, prepared by Others. G. Testing: The Contractor shall secure the services of an approved independent testing laboratory to conduct all required testing on sub -grade, base, asphalt and concrete. Locations required for these tests shall be as required by the City of Okeechobee, and/or in the case of the tum -lane improvements as required by the City of Okeechobee. Ata minimum, testing shall be as recommended by FOOT. Should any tests fail, contractor shall at his own expense, repair the deficiencies and retest the work until compliance with the specifications is demonstrated H. Traffic Control: The installation of Traffic Control Devices shall be in conformance to the requirements of the Manual of Uniform Traffic Control Devices, The City of Okeechobee. Drainage Improvements: All labor, materials and construction methods shall be in conformance to the minimum engineering and construction standards of the City of Okeechobee and FDOT Specifications. Trench excavation and back -filling operations shall meet or exceed the requirements of FDOT Specifications, Section 125. The Contractor shall provide the necessary back -fill compaction testing required to demonstrate compliance with this section. The pipe trench shall be dry when pipe is laid and the pipe shall be bedded per the details and per FDOT specifications. The Contractor shall comply with Chapter 90-96, Laws of Florida, which requires the Contractor performing trench excavations over five feet in depth comply with all applicable trench safety standards and shoring requirements as set forth in the Occupational Safety and Health Administration's (OSHA) ezcavation and safety standards, 29 C.F.R. 19926.650, Sub -part P and incorporated as the State of Florida standard, as revised and/or updated. The cost of compliance with this requirement shall be included as a separate line item on the Contractor's bid. Otherwise, Contractor certifies that the cost or compliance is included in the unit cost of all items of work to which this requirement applies. A. Reinforced Concrete Pipe (RCP): RCP shall conform to the requirements of ASTM Specifications C-76, Class III, Wall Thickness "B", latest revision. Contech A-2000 PVC drainage pipe (A-2000): All A-2000 Corrugated pipe with a smooth interior shall conform to the requirements of ASTM Designation F949 & F794 Dual Wall Corrugated Profile (DWCP) Pipe. Pipe and fittings shall be homogeneous throughout and free from visible cracks, holes, foreign inclusions or other injurious defects. Pipe shall be`' manufactured to 46 psi stiffness when tested in accordance with ASTM Test Method D2412. .`"� There shall be no evidence of splitting, cracking or breaking when the pipe is tested per ASTM Test Method D2412 and F949 section 7.5.
