MINUTES — Secretary. Building Official Newell moved to dispense with the reading and approve the Minutes for the regular meeting held January 19, 2017, seconded by Fire Chief Smith. Motion carried unanimously. V. UNFINISHED BUSINESS — Secretary. A. Secretary Burnette read into the record Form 8B Memorandum of Voting Conflict for County, Municipal, and other Local Public Officers related to Abandonment of Right -of -Way Petition No. 17- 001-SC which was received on January 19, 2017, and filed by Administrator Montes De Oca, address PO Box 1001, Okeechobee, Florida, and made part of the official record filed in the permanent Minute File for this meeting. VI. NEW BUSINESS — Chairperson. A. Site Plan Review Application No. 17- 001 -TRC, in regards to the construction of a 15,000 square foot, 18 bed West expansion, a 7,200 square foot laundry building and a 2,600 square foot building for additional kitchen facilities for an existing nursing home facility located at 1646 U.S. Highway 441 North, Okeechobee, Florida - Senior Planner. TRC - April 20, 2017 - Page 1 of 4 VI. NEW BUSINESS CONTINUED. A. Site Plan Review Application No. 17- 001 -TRC continued. The property owner obtained site plan approval for Application No. 15- 004 -TRC on July 16, 2015. The West parking area, and highway entrance expansion was completed. However, the remainder of the project has been modified; this application will supersede the previous one approved. Planning Staff Report: Planner Brisson began by commenting that the site plan submitted was confusing and required several conversations with the Project Engineer for clarification. The Okeechobee Health Care Facility is located on 10.95 acres, comprised of four unplatted parcels; including two annexed in April 2015. These four parcels are located as Parcel one 2.965± acres, is vacant and the northernmost parcel, lying East of Raulerson Hospital and West of Taylor Creek. Parcel two is 2.035± acres, is vacant and lies directly South of Parcel one, bounded by Parcel three to the West, Taylor Creek to the East, and Northeast 14th Street to the South. Parcel three is 4.319± acres, containing the majority of the facility, it is bounded by Parcel four to the West, Parcel two to the East, Raulerson Hospital to the North, and Northeast 14th Street to the South. Parcel four is 1.628± acres and houses the recently construed West parking area, it is bounded by U.S. Highway 441 North to the West, Parcel three to the East, Raulerson Hospital to the North and Northeast 14th Street to the South. The existing facilities encompasses approximately 81,127 square feet to operate a 180 -bed nursing home with administrative and staff offices, kitchen, dining, social, physical therapy and laundry areas, required parking (145 spaces), loading zones, retention, and landscaping. The project is proposed to be completed in phases. Each phase will appropriately address the required landscaping. paving and parking regulations as provided on the drawings. Phase One consists of a 14,700± square foot building which will expand the facilities by 18 -beds known as the "West- Expansion," and create a trash compactor area, to be located on Parcel four. Construct a 7,200 square foot laundry facility and loading dock to be located on Parcel two. Construct a 42,562 square foot dry retention area on Parcel one. Phase Two consists of expanding the kitchen by 2,600 square feet on Parcel three. Construct a 16,000 square foot facility on Parcel two that will expand the facility by another 18 -beds, and offices for staff, to be known as the "East- Expansion." Then on Parcel one, construct two 5,000 square feet and one 7,200 square foot maintenance buildings. Planning Staff's responses to the required findings are the property is designated Commercial on the Future Land Use Map and Heavy Commercial on the Zoning Map. Adequacy of public facilities appear to have been or will be met. Regarding potable water and sanitary sewer, the applicant had previously provided letters from the Okeechobee Utility Authority (OUA) documenting there is adequate capacity and facilities. The County has confirmed a considerable level of excess capacity available to serve the solid waste disposal needs. The Applicant has supplied an engineer's report addressing drainage and other aspects of the proposed project, and no sidewalks are required. Planning Staff noted a few items related to traffic generation and parking considerations in that the project will not have an effect on the level of service on U.S. Highway 441. However, should any parking space(s) or areas at the existing main facility that were devoted for laundry and /or maintenance activities change for other purposes such as additional beds, medical offices or out- patient services, such changes will need approval by the TRC or addressed through a Special Exception. TRC - April 20, 2017 - Page 2 of 4 VI. NEW BUSINESS CONTINUED. A. Site Plan Review Application No. 17- 001 -TRC continued. Additionally should the East - Expansion be used for any purposes by medical staff other than those directly associated with the nursing home, the site plan may have to be revised to address traffic generation and parking needs. The comparison of proposed and required dimensional standards found that most areas are adequate. Those that were not met were highlighted and listed as follows: 16 feet separation required between the two 5,000 square foot maintenance buildings, and between the new laundry facility and the East - Expansion buildings. Off- street parking, 241 total are planned, with eight being labeled handicapped. At least one handicap space is to be provided to serve the facilities proposed for Parcels one and two. A fourth loading zone is required and must be shown on the site plan. The driveway width for the parking aisle to the four parking spaces shown for Phase One on Parcel three has to be increased from 20 to 24 feet wide. The applicant needs to provide a specific landscaping plan suitable for scaling, so that staff can confirm the square footage of landscaped area. A photometric lighting plan conforming to City Code Section 78.71(A)(5) is to be submitted and reviewed. Mr. Steven Dobbs of SLD Engineering, LLC, was present and responded to various questions, noting the impervious information was included on the plans for Phases One and Two so that it would not have to be re- addressed. The drainage required for both Phases will be constructed in Phase One. The Architect is making final adjustments to the buildings, and once completed, he will be able to provide a site plan showing the crosswalks and striping. He confirmed a South Florida Water Management District permit was obtained. Administrator MontesDeOca noted as well that the proposed site plan was very confusing and asked several questions to clarify what would be built with which Phase and its location. He requested Mr. Dobbs provide water line information and potential looping for pressure, the location of fire hydrants and backflow prevention devices for future projects. Finally, that a traffic impact analysis is now needed based on the current and proposed uses. County Environmental Health Department: No issues were received. OUA: Director Hayford requested plans with the existing water and wastewater mains, a breakdown of employees listing existing, Phase One, Phase Two, and at peak demand. Provide water fixture counts on Phase one buildings in order to provide separate meters for each building, and to determine whether an easement is required. Noting the OUA and the Health Care Facility's Attorneys are researching to determine who owns the existing water main and whether an easement is required to maintain it since the entrance to the facility is a private drive. Public Works: No issues were received. Police Department: No issues were received. Building Official: Mr. Newell also asked questions to clarify his understanding of the specific proposed construction for each Phase and on which parcel. Fire Department: The age and condition of the fire service water main came into question based on the future projects that will require additional fire sprinkler protection and the ability of the system to provide a constant water source for fighting fires. Site Plan Review Application No. 17- 001 -TRC continued. not looped, which means that a damaged line could shut down operations while a looped system would at least have the possibility of water being supplied from a different direction. A fire engineer submitted with the previous site plan review, a letter confirming a looped system is not needed. Chief Smith stated an 8 -inch main is needed through the entire property. He also addressed that landscaping be kept trimmed in a manner to prevent low hanging branches across the driveway to the facility. In closing, Chief Smith added a safety concern related to the location of the Raulerson Hospital helicopter pad to the nursing home's driveway /access. His recommendation was some sort of system or device to prohibit vehicles from accessing the driveway from the facility's parking areas or from North Parrott Avenue, during the periods of time when emergency helicopters are landing or taking off. Chairperson Montes De Oca asked whether there were any comments or questions from those in attendance. There were none. He then asked TRC Members to disclose for the record whether they had spoken to anyone regarding the application or visited the site. There were no disclosures. A motion and a second was offered by Building Official Newell and Public Works Director Allen to have the applicant resubmit for the June 15, 2017 TRC Meeting for Site Plan Review Application No. 17- 001 -TRC. Committee Members are to compile their individual stipulations, email them to Board Secretary Burnette to forward to the Applicant for revisions and /or additional documentation to be submitted to General Services by May 18, 2017. Abandonment of Right -of -Way Petition No. 17- 001 -SC, a 200 by 60 -foot portion of Northwest 10th Avenue between Northwest 7th Court and 8th Street - Senior Planner. Before discussions began, Administrator Montes De Oca disclosed for the record due to a conflict that would inure a special private gain, he would abstain from discussion and voting on this item. The applicant, Mr. Jeremy LaRue, is a client of MDO Engineering, owned and operated by Mr. Montes De Oca. Planning Staff Report: Planner Brisson explained Mr. LaRue is requesting to abandon 60 feet of the 70 -foot wide unimproved right -of -way of Northwest 10th Avenue from the south boundary line of TRC - January 19, 2017 - Page 1 of 3 V. NEW BUSINESS CONTINUED. A. Abandonment of Right -of -Way Application No. 17- 001 -SC continued. 8th Street to Northwest 7th Court for a distance of 200 feet not including the intersection of 7th Court and 10th Avenue. The applicant owns Lots 1 to 6 and 15 to 20 of Block 13, NORTHWEST ADDITION, located immediately west of the subject property. Planning Staff's responses to the required findings are the property is designated Industrial on the Future Land Use Map and the Zoning Map. It is not the sole access to any property. Other properties bordering the proposed abandonment gain access by Northwest 7th Court, 7th Street, or 10th Avenue. The public benefits associated with the abandonment are, the existing right -of -way is currently unused and has no immediate or future use. It will be a benefit to the City to remove the existing right -of -way which will reduce the land the City is required to maintain and the property will be subject to ad valorem taxation. A utility easement is necessary for the Okeechobee Utility Authority (OUA) for existing infrastructure with 24 hour, seven days a week access. Based on these findings, Planning Staff is recommending approval. Mr. LaRue was present and added he paved the portion of Northwest 10th Avenue from Northwest 7th Street to 7th Court as required when developing his property. A stop sign was installed by the Public Work's Department. A fence was constructed around his property as well as the portion of Northwest 10th Avenue across the actual right -of -way. He will provide a combination lock to the City and OUA. Additionally, he has made a request for a street light to be installed at the corner of Northwest 7th Court and 7th Street for safety purposes. County Environmental Health Department: No issues were received. OUA: In the absence of Director Hayford, Secretary Burnette read into the record the OUA's comments provided on the petition. If the applicant is willing to give to the OUA an easement with 24 hour, seven days a week access, then the OUA is agreeable to the abandonment. The applicant and an OUA representative have met to "field locate" the OUA's line. Public Works: Director Allen commented that the City would require an easement to maintain the drainage ditch along the east side of the unabandoned portion of Northwest 10th Avenue. Police Department: No issues were received. Building Official: No issues were received. Fire Department: No issues were received. Project Description: The previously permitted east expansion is 7.68 acres in size will add an additional drainage basin to the west of 1.13 acres which will include the proposed building expansion of 18 beds as well as the existing parking and drive area. This will bring this basin to a total of 8.81 acres and is located in Sections 9 and 10, Township 37 South, Range 35 East. It is located in the City of Okeechobee, on the east side of SR 441N, north of the City Limits Ditch, on the following parcels (Parcel ID 2-09-37-35-0A00- 00010 -0000 owned by Lifestyles & Healthcare, LTD, 2- 10- 37- 35 -0A00- 00001 -B000 and 2-10-37-35-0A00- 00001-0000 both owned by OHCF Properties, LLC, and a portion of 2- 09- 37- 35 -0A00- 00010 -A000 to the west owned by Seniors "R" Able). The site is currently partially developed. There is a nursing home onsite with the only storm water system for this portion of the project is two existing culverts that discharge into the City Limits Ditch as shown on the construction drawings. The west basin was previously permitted and developed as a parking area and does drain through a detention system that provides water quality and attenuation for that portion of the project. The drainage calculations and routings to address the required water quality treatment volumes and anticipated surface water stages for the proposed site improvements will be provided. The surface water management system for the proposed Nursing Home Expansion will consist of a new dry detention area. The control structure for the new dry detention area will connect via existing culvert on the adjacent property to Tylor Creek to the east. Additionally, a perimeter berm will be constructed that is sized to retain the 25 year, 3 day storm event on the site, around the east, north and south sides of the property with the access road along the south side of the property acting as a perimeter berm. Proposed Use: The project consists of an existing nursing home, associated parking, and an existing exfiltration trench which will be removed with this project. The proposed improvements consist of an additional parking, multiple building expansions in various phases and construction of a dry detention area. Water quality and water quantity requirements will be met in the dry detention areas. Potable water and wastewater service is provided by the Okeechobee Utility Authority. The planned expansion will incrementally increase the existing potable water and wastewater demand. Drainage Considerations: To attenuate the runoff generated by the existing and proposed improvements and to meet water quality requirements, all drainage will be routed through the dry detention area. The dry detention area will be controlled at elevation 18.2 NGVD `29 which is the required elevation to meet the nutrient removal goal for this project described later in this document. This portion of the project will discharge through a proposed new discharge location into the Airport Ditch to the south. The Soils Map for Okeechobee County indicates that this parcel's soils are made up of Immokalee Fine Sand and Manatee Loamy Fine Sand, Depressional soils, which is a very poorly drained "D soil" in the natural state and a better drained `B soil" in the developed state. The Soils Map information also indicates that the anticipated wet season water table is approximately 0" below natural ground for Manatee Loamy Fine Sand, Depressional soils and approximately 1' below natural ground for the Immokalee Fine Sand. However, since this site is adjacent to Taylor Creek and as established above the discharge elevation for this project is going to be 17.0 NGVD '29, the bottom of the dry detention area will be constructed at 18.0, the proposed discharge elevation of 18.2 is proposed to meet water quality requirements for the project The site is located in the S -133 Basin. The proposed outfall for the project will be on the south side of the proposed dry detention area that will discharge through a proposed culvert that will discharges into the Airport ditch to the south then into Taylor Creek located to the east of the Airport Ditch. (Taylor Creek ultimately discharges into Lake Okeechobee.) 2 The allowable discharge for the S -133 Basin is 15.6 csm (cfs /sq. mile) for the 25 year 3 day event: Q = 15.6 csm / 640 acres /sq. mile x Project Area Q = 15.6 csm / 640 acres /sq. mile x 8.81 acres = 0.21 cfs A. Water Quality Water quality treatment is provided in the form of dry detention for the project drainage area. Since the proposed water quality system is dry detention, the volume required is 75% of the calculated volume, however since this project discharge into an impaired water basin and with a presumption of compliance with nutrient control by adding an additional 50% to the water quality volume the total water quality volume is 0.98 acre -feet. The dry pretreatment criteria of (0.5 ") is less than the (1 ") water quality requirement, therefore the system meets the pretreatment criteria as specified under Section 5.2.1 of the SFWMD's Environmental Resource Permit Information Manual Volume IV and the required water quality volume. Based on the attached stage storage spreadsheet, the water quality volume (0.98 acre -feet) is met at elevation 19.19 NGVD '29. B. Water Quantity This project is located in the S -133 Basin which discharges ultimately into Lake Okeechobee. The allowable peak discharge rate in this basin is 15.6 csm. The allowable peak discharge rate for this project, based on the 25 -year, 72 -hour storm event was calculated to be 0.21 cfs. The actual maximum discharge rate for the 25- year, 72 -hour storm event was calculated to be 0.387 cfs, which is greater than maximum allowable peak rate, but discharging through the proposed 3" minimum bleeder. To demonstrate conformance to this criteria, the proposed project was flood- routed using AdICPR. The 10 year, 24 hour storm (5.0 ") w/ discharge, the 25 year, 72 hour storm (9.0 ") w/ discharge, and the 100 year, 72 hour storm (10.0 ") w/o discharge, were evaluated based on the proposed plan. Please refer to the attached AdICPR flood routing input/output parameters. A summary of the flood routings is provided as follows: 10 Year, 24 Hr. Storm (5.0 ") 25 Year, 72 hr. This expansion is for 198 total beds in the nursing home and 94 employees at peak shift requiring 144 parking spaces onsite. There are currently 116 existing parking spaces and 28 planned parking spaces, which will meet the parking requirements. There will be 10 handicapped parking spaces around the facility, which exceed the ADA required 6 spaces per 150 parking spaces provided. Off -Site Drainage: There is an existing offsite flow from the hospital to the north. The discharge is located on the west side of the north parcel and discharges into the southwest portion of the wetland. When this discharge was originally permitted, there was a drainage easement grated by the Hamrick parcel to allow the discharge through the property. When this parcel was purchased by the nursing home, they took exception to the drainage easement and had the Hamricks move the easement to the north off the parcel. In this process, the easement was moved, but the discharge structure was not. The hospital has received a permit to relocate the drainage structure, but have not done so as of this submission. There are impacts proposed in the wetland. Nutrient Analysis: As previously stated, the project proposes to provide 150% of the required water quality treatment volume in the dry detention system in order to meet the nutrient removal requirements. In addition, a nutrient analysis was completed using BMP Trains Model version 7.4, which indicates an additional 0.068 acre -feet of retention are required to meet the nutrient load reduction. This will be achieved discharging at elevation of 18.2 NAVD '88, which gives an additional 0.11 acre -feet of retention. With this additional retention, the system will be left at elevation 14.4 inches above wet season water table. From the soil survey, the K -sat is 28 micrometers per second or 4" per hour. This would require less than 4 hours to recover to wet season water table. Flood Plain Analysis: As shown on the attached FEMA Panel 12093C0415C, the eastern portion of this project is located in a flood zone AE el. 16.0 NAVD '88. However, existing land is above this elevation thereby, providing assurance potential flooding of this facility is minimal. Irrigation: This site will be irrigated with a new well onsite. A SFWMD Water Use Permit will be applied for under separate cover. Construction Recommendations: Runoff and /or any water generated by short-term dewatering during construction will be contained on -site. However, there is some potential for transport of sediment to off -site areas should heavy rainfall occur. In order to reduce the potential of any off -site transport of sediment or turbidity we recommend installation and maintenance of temporary silt fence inside the southerly property line in the area of the proposed paving of the existing rock road and upstream of the existing control structure until site work has been completed and the site has been stabilized. (The existing perimeter berm should prevent any potential off -site transport of sediment from other areas of the site during construction.) Conclusions: In my professional opinion, the proposed construction should have no impact to existing drainage patterns off -site and should have no impact on off. site areas. The recommendations above should be followed during and after the site work until such time as the ground surface has been adequately stabilized to prevent the off -site transport of any soil or suspended solids. The Applicant previously received approval in 2015 of a site plan for the "East Expansion" of the nursing home facility, including the two easternmost parcels which were annexed into the City to facilitate the expansion. That approval also included the two 5,000- square foot maintenance buildings to be located on Parcel 1 in Phases 2 and 3. The overall site plan now includes a 7,200- square foot maintenance building on Parcel 1, and a 16,000 - square foot building on Parcel 2 associated with 18 additional beds and nursing and doctors' office space, expanding the nursing home facility to 180 beds. The new site plan is proposed to include a 15,000- square foot, 18 -bed "West Expansion" increasing capacity to 198 beds. The site plan also includes a 7,200- square foot laundry building and a 2,600- square foot building devoted to expanded kitchen facilities. There are 71 parking spaces around the existing nursing home facility. Another 74 spaces were recently approved on the lot immediately west of the existing facility. This parking lot also includes the addition of a trash compactor that will handle all of garbage generated by the facility. The existing garbage dumpsters will be removed. The City of Okeechobee requires one parking space for every four beds and one space for every employee at peak shift. The proposed expansion will increase the number of beds to 198 in the nursing home. There will be 94 employees at peak shift, requiring 144 parking spaces onsite. There are currently 16 existing parking spaces on Parcel 3 and 128 more planned for Parcels 1 and 2. There will be 10 handicapped parking spaces around the facility, exceeding the ADA requirement of six spaces for a 150 -space parking facility. The total area of the four parcels comprising the site is 10.95 acres. The total building area of structures associated with the proposed new development is 2.58 acres, resulting in a lot coverage of 23.6 %. Of the total acreage, this submittal is for a drainage area of 8.81 acres of which 5.44 acres is impervious or 61.7% appreciably less than the maximum 85% allowable. The applicant is planning to use the existing access to the site from the west which was previously expanded through FDOT to a 16 -foot wide entrance lane and a 12 -foot wide exit lane. SOLID WASTE DISPOSAL: On several occasions the County has confirmed a considerable level of excess capacity available to serve the solid waste disposal needs of other developments in the City. It appears reasonable that the volume of solid waste associated with a development of this size can also be accommodated within the capacity of the County's Solid Waste Facility. DRAINAGE: The Applicant has submitted, as part of the Application submittal, an Engineer's Report for the West Wing Expansion prepared by Steven L. Dobbs Engineering, LLC which addresses drainage and other engineering aspects of the proposed project. TRAFFIC GENERATION AND ACCESS CONSIDERATIONS: TRAFFIC GENERATION: The traffic generation factor for a nursing home cited in the ITE 8th Edition of Traffic Generation is 0.22 trips per bed in the PM "peak hour ". This translates to an additional four trips during the PM Peak Hour for the 18 new beds being added in the planned expansion. It is safe to say that this increase in traffic will have no effect on] the level of service on US 441. Since the proposed new laundry and maintenance facility buildings and expansion of the kitchen facilities are intended to accommodate activities currently being conducted in the main building or elsewhere, no additional traffic is expected to result when these buildings are constructed in the future. However, when the laundry and maintenance activities are transferred to the new facilities, should any space devoted to these activities in the existing facility be used for other purposes (e.g. additional beds, medical offices, or out - patient services) such changes will need Special Exception and /or site plan approval at that time, as they may increase the intensity of the use.
