0011 (Vital Statistics)ORDNANCE NO. 11. AN ORDINANCE PROVIDING FOR THE REGISTRATLCN OF BIRTHS AND DEATHS, THE KEEPING OF PROPER RECORDS THEREOF AND THE REPORTING OF THE SAME TO THE STATE BOARD OF HEALTH, AND PRESCRIBING PENALTIES FOR VIOLATIONS OF DUTIES !{REIN PROVIDED IN CONNECTION IHEREWITj1 . BE IT ORDAINED BY TIE MAYOR AND CITY COUNCIL OF THE CITY OF OKEEC OBEE, FLORIDA: SEC. 1.That the City Clerk shall be local registrator of vital statistics, and it shall be his duty to supply to all physicians, midwives, un- dertakers and others requirfing them for use, blank certificates for the registration of births and deaths, which shall be of the standard form approved by the -State Board of Health, and tor eceive, and_make copies of such certificates, and transmit the originals to the State Board d' Health on the tenth (loth) day of each month, and to issue burial, removal or other permits upon the receipt of complete, satisfactory and properly executed certificates of death; and no dead body of a human being whose death occurs, or whose body is found, in this City shall be interred, otherwise disposed, of, removed from the City, or held more than seventy two hours after death, w ithout a burial, removal or other permit issued by the City Clerk in advance of such interment, other dis- position or removal. The undertaker or person acting as such to obtain the certificate of death, have it filled out with r espect to the personal particulare, present it to the attending p »sician for the medical certificate of =arc cause of death, and obtain the burial, removal or other permit taws from the City Clerk; provided, that when there was no attending mut physician, the certificate shall be referred to the health officer or coroner for statement of cause of death. 3. That it shall be the duty of the physician, midwife, or person i,ng as midwife, a tteiiding a case of confinement in this City to file a coplete, satisfactory and properly executed certificate of birth with theCity Clerk within three (3) days thereafter; provided, that when there was no physician or midwife in attendance, then it shall be the duty of the parent, householder, or owner bf the premises where the birth occured to file such certificate. SEC. 4. That any physician, midwife, undertaker, or other person who violates or fails to comply with any of the provisions or duties of this Ordnance, or any section thereof, shall upon trial, and conviction thereof be punished by fine not exceeding $100.00, or by imprisonment not exceeding sixty days, or by both such fine and imprisonment. SEC. S. This Ordinance shall b e in full force and effect on and after its massage and approval b7 the Mayor.
