All electrical work shall be performed by a licensed electrical contractor. The completed system shall be fully operative,. All work shall be done in accordance with the N.E.C. and shall comply with all local codes and ordinances, and shall New meet all standard requirements of the local electric utility. 8. Contractor shall guarantee all materials and workmanship free of defects for a period of not less than one year from the date of acceptance or certificate of occupancy. Correction of defects shall be completed without additional charge and shall include replacements or repair of any other phase of installation that may have been damaged. Noir -3- Scope of Work: 1. Installation of a pre fabricated vertical lift, per manufacturers specifications. This shall include structural modifications that may be required at the foundation (i.e. construction of a pit), and all electrical work necessary for a completely operation system. Minor alteration to the existing opening afthaaroposed door location may_ be required. 'tv r4- 2. The removal and relocation of electrical components adjacent to the proposed lift 4 location,. This shall include any augmentations to said components as may be required by N.E.C. or the current building code. 3. The installation of a new door as indicated on the building layout. This shall require the creation of an opening in the existing masonry (load bearing) wall, as tihIVr y detailed on sheet S-2. Addition to the existing concrete pad shall be necessary to accommodate the required landing area (stoop)at the new doorway. 4. The parking area shown on the building layout shall need to be re-striped in accordance with local code and ADA standards. Details are on sheet W-2. 5. An existing 2nd floor window opening shall be altered to allow for the vertical lift doorway. Structural modifications may be required, depending upon the pre fabricated lift specifications. New 6. Alterations to the existing metal cover at the intended work area shall be required. This includes removal of the cover where it interferes with the proposed lift, and the fortifying of remaining elements. -4- Licenses: .• Vendors shall have a current City/County occupational license and/or regulatory permit, or be willing to obtain whichever is applicable. Inspection of Facility: Bidders who are going to submit a proposal must first, visually inspect the site where the items/services are to be provided/installed/performed. An appointment for inspection shall be made with the City's designee (see attached inspection forms). Failure to visually inspect the site(s) and get inspection form(s)signed by the proper person, shall be cause for disqualification of your bid. It is the bidders responsibility to submit signed inspection forms with their bid. Bidders are advised to make a thorough inspection of the site and the extent of work required to fulfill this bid. After the bid has been awarded, no extra charge or compensation will be allowed. Qualification of Bidders: This bid shall be awarded only to a responsible bidder, qualified to provide the work specified. The bidder shall submit the following information with his proposal: A. Experience record showing the bidder's training and experience in similar work. B. List brief description of similar work satisfactorily completed with location, dates of contracts, names addresses, and phone number of the owner, or ,,. inspector of work performed. C. The City reserves the right to reject any and all bids with or without cause. Cancellation: This contract will be subject to immediate cancellation if either product or service does not comply with specifications as stated herein or fails to meet the City's performance standards. Prior to such cancellation,the City will provide vendor five (5)days time to correct or cure any defect. Equipment: It shall be the vendor's responsibility to ensure that all equipment used shall be properly maintained in order to protect the operator and public/private property. Any equipment that is inoperable, unsafe, or improper for desired use shall be removed from the premises. The City's representative(s)will have the right to reject the use of any specific piece of supplied equipment on site. It must be immediately removed and another unit used to perform the tasks. All mowers are to be adjustable and adjusted to the proper cutting height and level for the kind of grass and current condition of the turf. All mower blades are to be sharp enough to cut, rather than tear the grass. -5- Insurance: r••- Vendor shall provide proof of general liability insurance coverage throughout the duration of this contract, in the following limits: Worker's Compensation As required by State Law. The address to which communications regarding the Bid are to be directed must be shown. Submission of Bids 1. Submit bids on Bid Forms supplied herein. Bids shall be submitted at the time and place indicated in the Call for Bids and shall be included in an opaque sealed envelope marked with the project title and name and address of the bidder and accompanied by the bid security. If the bid is sent through the mail or other delivery system,the sealed envelope shall be enclosed in a separate envelope with the notation "Bid Enclosed"on the face thereof. 2. Bids received after the time and date specified in the call for bids shall not be considered and will be returned unopened. -6- Modification and Withdrawal of Bids �... 1. Bids may be modified or withdrawn by an appropriate document duly executed (in the manner that a bid must be executed) and delivered to the place where bids are to be submitted at any time prior to the opening of bids. Withdrawal of a bid will not prejudice the rights of bidder to file a new bid. Opening of Bids 1. At the specified time and place as indicated in the call for bids, bids shall be opened publicly and read aloud. 2. Not withstanding any other provision contained in the contract documents, the City reserves the right to reject any and all bids, or portions thereof, and waive any and all irregularities, and the right to disregard any or all nonconforming, unbalanced, or conditional bids or counter proposals. The City may reject, as non responsive, any or all bids where bidders fail to acknowledge receipt of addenda as prescribed. If the City elects not to reject a bid which fails to acknowledge receipt of any addendum, the bid shall be construed as though the addendum has been received and acknowledged by the bidder. For basis of award, discrepancies between the total bid and the sum of columns of figures shall be resolved in favor of the total bid. Contractor shall be responsible for the accuracy of the arithmetic of the bid in particular with the addition of the columns to match the total bid. City of Okeechobee has the right to accept the total bid whether it is less or more than any additions of any columns or extensions, and prorate the unit prices to match the total bid amount. Contractor shall loose his bid deposit if contract award is not accepted by such contractor. Bids to remain open. 1. City may conduct such investigations and require supplemental information as it deems necessary to assist in the evaluation of any Bid and to establish the responsibility, qualifications, *ow and financial ability of the bidders, proposed subcontractors, proposed equipment and material suppliers, and other persons and organizations to do the work in accordance with the contract documents to the City's satisfaction within the prescribed time. 5. The City reserves the right to reject the bid of any bidder who does not pass any such evaluation to the City's satisfaction. 6. City reserves the right to reject the bid of any bidder which is or was behind on the approved completion schedule for any existing or past contracts with the City, is in litigation with the City, has previously defaulted on a contract with the City, has delayed or skipped subcontractor payment(s), or has provided unsatisfactory performance on current or previous City contracts. 7. If the Contract is to be awarded, it will be awarded to the best and lowest responsible bidder on the basis of the total bid amount. The City reserves the right to reject all bids or portions thereof. The City also reserves the right to accept and award on an item by item basis. Signing of Agreement 1. When City gives a Notice of Award to the successful bidder, it will be accompanied by at least two (2) unsigned counterparts of the agreement and all other contract documents.
