RE: Mosquito Control
I reported that we have spent about $300 to date on mosquito control dunks,
applying them throughout town in any standing water. Research indicates
that that application is most effective on future eradication, but, of course,
does very little to arrest existing flying mosquitoes.
On Monday, Oct 15th, David and I met with Dug Wilson, Fleming Pest
Control, and I've attached a quote hereto. It's derived from the Clewiston
contract.
Please add to the Agenda a motion to piggyback on Clewiston's
Mosquito Control Contract retaining Fleming Pest
Management, Lehigh Acres, to conduct an adult mosquito
insecticide application.
Particulars include at least one trap setting ($75) and insecticide
application at the rate of$20.15/spray mile with an estimate of 65
miles for a total of about $1300, and purchase two cases of
mosquito dunks at a rate of$86.48 @, equaling about $200 for a
total of about $1600 in mosquito control.

1. Purpose of Agreement
The City has provided a Request for Proposals to contractors and has received sealed bids
for complete mosquito control. including but not limited to. Aerial Adult Mosquito Insecticide
Applications at $6.010.00 per application, Ground Ultra Low Volume (ITV) Adult Mosquito
Insecticide Applications at $20.15 per spray mile. Mosquito Larva Slow Release 120-day,
Larvicide Applications at $74.40 per acre, Mosquito Larva Slow Release (Briquette) 120-day.
Larvicide Applications in Catch Basins at $6.20 per briquette. Sticky Spray Migrration Barrier
Adult Mosquito Insecticide with a Minimum of a Seven (7) Day Effectiveness at $72.00 per
spray mile, and Adult Mosquito Trapping, Monitoring, and Species identification at $10,400.00.
The purpose of this agreement is to set forth the terms and conditions for the award of this
contact to the Contractor to perform those services in accordance with the TECHNICAL
SPECIFICATIONS attached hereto and incorporated as an integral component of this agreement.
The Bid Document entitled ``The City of Clewiston Professional Aerial and Ground Control
Services" presented by Fleming Pest Management, Inc. is incorporated as an integral component
of this agreement. The services are to be provided on the areas within the 'Harlem Community
and the City of Clewiston.
2. Initial Annual ',Perm of Agreement
Except as hereafter provided, this agreement shall commence on October 1. 2010, and
shall end on November 30. 2011.
3. Renewal of Contract and Subsequent Terms
The contract term, inclusive of all other terms and conditions provided herein, may be
extended beyond the Initial Annual Term for up to two (2) additional annual terms. provided the
Contractor requests and the City Manager grants in writing an extension for an additional annual
term(s). 13-1 no event shall the terms and conditions of this agreement last beyond November 30.
2013.
4. Termination of Contract — Without Cause

6. Obligations of the Contractor

A. Perform all obligations pursuant to this contract limited to obtainir 5` and maintaining
in effect all applicable licenses_ permits. and \�'urk is" Compensation Insurance, and
H. Maintain throughout the terrnts) of the contract general public liability insurance and
vehicular insurance policies having personal injury limits for not less than limits of
liability mandated by law, which policy or policies must also name the City as en-
insured. Contractor must provide the City with a certificate evidencing such insurance
within three (1) business days after the execution of this contract, which certificate
shall contain a provision that the coverage cannot he canceled without sixty (60) days
prior wwritten notice to the City;
C. Provide all of the necessary personnel, vehicles, equipment, supplies, fuel, chemicals,
and materials to perform all of the services specified in this contract, and to perform
all of those services in a professional and timely manner,and
D. Provide the City with a copy of a detailed listing of tell personnel and a detailed
inventory of all equipment necessary to demonstrate that the Contractor can
adequately perform the contract. That listing and inventory are attached hereto and
incorporated as an integral component of this agreement.
E. Maintain all equipment used in performing the contract in good working condition
and neat in appearance with all safety guards in place;
▪ Perform all of the obligations of Contractor pursuant to this agreement to the
reasonable satisfaction of the City Manger of the City, or his expressly appointed
designee.
G. By the 7th day of each calendar month during the term(s) of the contract deliver to
City Manger of the City, or his expressly appointed designee, a statement of the
services provided by Contractor during the preceding calendar month_
7. Obligations of the City
Throughout the term of the agreement,the City shall:

Good Morning Mr. Allen,
It was very nice meeting with you and Mr. Whitehall yesterday. Attached is a copy of the
Clewiston contract pricing, (it has been extended) Fleming Pest Management will extend
contract pricing to the City of Okeechobee for all services requested. The light trapping
(surveillance) is a lump sum on the contract, but broken down it works out to be $75.00 per trap
setting. All Fleming needs to begin treatments on an emergency basis is approval and any maps
the City may have showing areas to be treated (roads, paths etc). Adulticde (road spraying) will
be performed by either truck or Kubota ATV at speeds less than 15 mph at a flow rate of 9.0
ounces per minute at 10 mph. This flow rate makes a large enough cloud to travel hundreds of
feet from the road into harborage areas.
If in the future the City needs to purchase Larvicides, please feel free to reduce your costs by
buying in bulk:
Mosquito Dunks (bti) $86.48 per case of 100
Altosid Briquets $142.50 per 100 (longer lasting)
